REQUEST FOR QUALIFICATIONS

MATERIALS TESTING & GEOTECHNICAL

Crowley Skate Park

Mono County is seeking a qualified consulting firm to provide quality assurance soils and construction materials sampling, testing, and laboratory services for the Crowley Lakes Skate Park.

All qualified firms interested in providing these services are invited to submit their Statement of Qualifications (SOQs). The Consultant’s SOQs will be evaluated and ranked according to the criteria provided in Appendix B, “Proposal Evaluation,” of this RFQ. Consultant shall provide their estimated costs and their rate schedules only after initial selection, and shall not include costs in the SOQ.

SOQ’s must be submitted prior to 4:00 PM, 2017 September 5.

Any questions related to this RFQ shall be submitted in writing to the attention of Paul Roten via email at proten@mono.ca.gov.  Questions shall be submitted before 4:00 PM on September 1, 2017.

Consultant shall refer to Appendix A for submittal requirements and to Appendix B for Evaluation Criteria.

SCOPE OF SERVICES

Consultant shall provide testing services for the Crowley Skatepark construction.  This work will include placement of approximately 350 cubic yards of concrete and shotcrete concrete. Installation of shotcrete concrete of approximately 8 to 16 cubic yards a day for 3 weeks, along with 1 week of concrete flatwork poured in place.  Additional work will include one day of paving for a parking lot of approximately 5,500 square feet. Contractor to provide shotcrete testing panels. Compression testing every 50 cubic yards is acceptable. Concrete mix testing to be performed more frequently, as needed, due to ready mix plant operations.

Quality assurance materials sampling and testing work consists of performing Local Agency or ASTM test methods. The frequency and testing requirements are found in the Mono County Quality Assurance Plan.

Specific Deliverables will be as agreed upon with Mono County

Schedule shall be coordinated with Mono County Public Works to meet the requirements of the Crowley Skatepark construction.
